BackgroundAttention deficit hyperactivity disorder is clinically diagnosed; however, quantitative analysis to statistically analyze the symptom severity of children with ADHD via the measurement of head movement is still in progress. Studies focusing on the cues that may influence the attention of children with ADHD in classroom settings, where children spend a considerable amount of time, are relatively scarce. Virtual reality allows real-life simulation of classroom environments and thus provides an opportunity to test a range of theories in a naturalistic and (...) controlled manner. The objective of this study was to investigate the correlation between participants’ head movements and their reports of inattention and hyperactivity, and to investigate how their head movements are affected by different social cues of different sensory modalities.MethodsThirty-seven children and adolescents with and without ADHD were recruited for this study. All participants were assessed for diagnoses, clinical symptoms, and self-reported symptoms. A virtual reality-continuous performance test was conducted under four conditions: control, no-cue, visual cue, and visual/audio cue. A quantitativecomparison of the participants’ head movements was conducted in three dimensions using a head-mounted display in a VR classroom environment. Task-irrelevant head movements were analyzed separately, considering the dimension of movement needed to perform the VR-CPT.ResultsThe magnitude of head movement, especially task-irrelevant head movement, significantly correlated with the current standard of clinical assessment in the ADHD group. Regarding the four conditions, head movement showed changes according to the complexity of social cues in both the ADHD and healthy control groups.ConclusionChildren and adolescents with ADHD showed decreasing task-irrelevant movements in the presence of social stimuli toward the intended orientation. As a proof-of-concept study, this study preliminarily identifies the potential of VR as a tool to understand and investigate the classroom behavior of children with ADHD in a controlled, systematic manner. Background: Poor psychosocial work environments are considered critical factors of nurses’ intention to leave their profession. Workplace injustice has been proven to increase the incidence of psychiatric morbidity among workers. However, few studies have directly investigated the effect of workplace justice on nurses’ intention to leave their profession and the population attributable risk among nurses. Objective: This study identified factors associated with workplace justice and nurses’ intention to leave the profession. Method: A cross-sectional survey was conducted using a self-administered structured (...) questionnaire. Approximately 10% of all secondary referral centers in Taiwan were stratified and randomly sampled. Multiple logistic regression and population attributable risks were preformed to assess the effect of workplace justice on nurses’ intention to leave the nursing profession. Ethical considerations: This study was approved by the Research and Ethical Committee of National Taiwan University Hospital. Only nurses who consented to the study participated in the survey. Result: A total of 2268 nurses were recruited, of whom 1417 (62.5%) satisfactorily completed the questionnaire. The participants were classified and 342 (24.1%) of them were placed into the low workplace justice group. Nurses with low workplace justice had a higher intention of leaving the profession (adjusted odds ratio = 1.34, 95% confidence interval = 1.02–1.77). “Employees’ opinions are influential in hospital’s decision making” and “employees’ performance is evaluated fairly” were the most influential factors of the participants’ intention to quit. The adjusted population attributable risk was 3.7% for low workplace justice. Conclusion: This study has identified that workplace justice is a protective factor of nurses’ leaving their current profession. A fair performance appraisal system and increased autonomy at work are warranted to dissuade nurses from leaving the nursing profession. (shrink)

We investigated whether, in the emergency department, educational video-assisted informed consent is superior to the conventional consent process, to inform trauma patients undergoing surgery about the procedure, benefits, risks, alternatives, and postoperative care. We conducted a prospective randomized controlled trial, with superiority study design. All trauma patients scheduled to receive trauma-related debridement surgery in the ED of Kaohsiung Medical University Hospital were included. Patients were assigned to one of two education protocols. Participants in the intervention group watched an educational video (...) illustrating informed consent information, whereas those in the control group read an informed consent document. The primary outcome was knowledge scores and the secondary outcome was assessment of patient satisfaction. A multivariable regression model, with predefined covariates, was used to analyze differences in knowledge scores and patient satisfaction levels between the groups. A total of 142 patients were enrolled, with 70 and 72 assigned to the intervention and control groups, respectively. Mean knowledge scores were higher in the intervention ) than in the control group. By multivariate analysis, the intervention group had significantly greater differences in knowledge scores. Age, injury severity score, and baseline knowledge score significantly affected the differences in knowledge scores. Significant improvements were observed in patients’ perception of statements addressing comprehension of the information provided, helpfulness of the supplied information for decision making, and satisfaction with the informed consent process. Multivariate analysis showed significant correlations between video education and patient satisfaction. Both the educational approach and severity of injury may have an impact on patient understanding during the informed consent process in an emergency environment. Video-assisted informed consent may improve the understanding of surgery and satisfaction with the informed consent process for trauma patients in the ED. Institutions should develop structured methods and other strategies to better inform trauma patients, facilitate treatment decisions, and improve patient satisfaction. Electroacupuncture (EA) and electroconvulsive therapy (ECT) are often used in the management of schizophrenia. This study sought to determine whether additional EA and ECT could augment antipsychotic response and reduce related side effects. In this retrospective controlled study, 287 hospitalized schizophrenic patients who received antipsychotics (controls, n = 50) alone or combined with EA (n = 101), ECT (n = 55) or both (EA+ECT, n = 81) were identified. EA and ECT were conducted for 5 and 3 sessions per week, (...) respectively, with a maximum of 12 sessions for ECT during hospitalization. The Scale for the Assessment of Positive Symptoms (SAPS) and Scale for the Assessment of Negative Symptoms (SANS) were used to assess the severity of psychotic symptoms. Clinical response on SAPS and SANS, weight gain, and adverse events were compared. Survival analysis revealed that the ECT and EA+ECT groups had markedly greater clinical response rate than controls on SAPS [72.7% and 90.1% vs. 64.0%; relative risk (RR), 1.974 and 2.628, respectively, P ≤ 0.004] and on SANS (67.3% and 70.4% vs. 42.0%; RR, 1.951 and 2.009, respectively, P ≤ 0.015). A significantly greater response rate on SANS than controls was also observed in the EA group (64.4% versus 42.0%; RR = 1.938, P = 0.008). EA-containing regimens remarkably reduced weight gain and incidences of headache, insomnia, dry mouth, and electrocardiographic abnormalities. These results suggest that EA and ECT can serve as additional treatment for enhancing antipsychotic response and reduce the side effects in hospitalized patients with schizophrenia. (shrink)

The human relation to things in the world is at stake in the so-called post-humanist era where the distinction between human and non-human is blurred, as indicated in a term like ‘the nano-self’. How should we understand the nature of our relation to things in this era? Or how can we describe an educationally meaningful relation we as human agents can make in relation to things, artificial and natural, in the face of this technologically hybrid and ever-dehumanizing tendency of society? (...) Well-known for his actor-network theory, Bruno Latour, an influential materialist and philosopher of technology, says that ‘objects’ have agency as much as humans; so we need to be able to translate the language of things into the language of men or vice versa as a way of co-shaping the world we live in. What does this mean and how can we do so? In carefully examining his theory, the essay attempts to explore and reformulate his account of the mediating role of human agency to see if it can provide us with an educationally plausible post-humanist concept of human agency, which can lead us into an ecologically ethical and politically responsible approach to education in general, and science education in particular. In this paper, we tackle the widely held view that practice-oriented approaches to ethics are conservative, preserving the moral status quo, and, in particular, that they do not promote any (fundamental) change in our dealings with animals or formulate clear principles that help us to achieve such change. We shall challenge this view with reference to Wittgensteinian ethics. As a first step, we show that moral thought and action rest on basic moral certainties like: equals are to be treated equally (...) and unequals unequally. We then explore the relations between this insight and the notion of the difficulty of reality elaborated by Cora Diamond. Our goal is to show that a Wittgensteinian-Diamondian approach to animal ethics is not necessarily morally conservative. Indeed, it offers a profound practice-oriented approach to animal ethics that is capable of promoting change in human dealings with animals because it is compatible with radical critique. (shrink)

Muscular dystrophies are associated with mutations in genes encoding several classes of proteins. These range from extracellular matrix and integral membrane proteins to cytoskeletal proteins, but also include a heterogeneous group of proteins including proteases, nuclear proteins, and signalling molecules. Muscular dystrophy phenotypes have also become evident in studies on various knockout mice defective in proteins not previously considered or known to be mutated in muscular dystrophies. Some unifying themes are beginning to emerge from all of these data. Obtaining adequate informed consent from trauma patients is challenging and time-consuming. Healthcare providers must communicate complicated medical information to enable patients to make informed decisions. This study aimed to explore the challenges of obtaining valid consent and methods of improving the quality of the informed consent process for surgical procedures in trauma patients. We conducted a systematic review of relevant English-language full-text original articles retrieved from PubMed that had experimental or observational study design and involved adult trauma patients. Studies involving (...) informed consent in clinical or research trials were excluded. Titles and abstracts of searched articles were reviewed and relevant data were extracted with a structured form. Results were synthesized with a narrative approach. A total of 2044 articles were identified in the initial search. Only eight studies were included in the review for narrative synthesis. Six studies involved orthopedic surgeries, one involved nasal bone surgeries, and one involved trauma-related limb debridement. Only one study was conducted in an emergency department. Information recall was poor for trauma patients. Risk recall and comprehension were greater when written or video information was provided than when information was provided only verbally. Patient satisfaction was also greater when both written and verbal information were provided than when verbal information alone was provided; patients who received video information were more satisfied than patients who received written or verbal information. Many articles have been published on the subject of informed consent, but very few of these have focused on trauma patients. More empirical evidence is needed to support the success of informed consent for trauma patients in the emergency department, especially within the necessarily very limited time frame. To improve the informed consent process for trauma patients, developing a structured and standardized informed consent process may be necessary and achievable; its effectiveness would require evaluation. Adequately educating and training healthcare providers to deliver structured, comprehensive information to trauma patients is crucial. Institutions should give top priority to ensuring patient-centered health care and improved quality of care for trauma patients. (shrink)

In this paper I discuss the ways in which the early Christian writer Arnobius of Sicca used rhetoric to shape religious identity inAduersus nationes. I raise questions about the reliability of his rhetorical work as a historical source for understanding conflict between Christians and pagans. The paper is intended as an addition to the growing literature in the following current areas of study: the role of local religion and identity in the Roman Empire; the presence of pagan elements in Christian (...) religious practices; the question of how to approach rhetorical works as historical evidence. Interpreting Kant's 'critical turn' as a normative turn, he argues that Kant's theory of normativity is both original and radical: it departs from the perfectionist ideal of early modern rationalism, and arrives at an unprecedented framework of synthetic a priori principles that determine the validity of our judgments. Pollok examines the hylomorphism in Kant's theory (...) of normativity and relates Kant's idea of our reason's self-legislation to the 'natural right' tradition, revealing Kant's debt to his predecessors as well as his relevance to contemporary debates on normativity. Cognitive enhancement has an increasingly wider influence on our life. The main issue that concerns epistemologists is what its epistemological implications are. Adam Carter and Duncan Pritchard argue that cognitive enhancement improves cognitive achievement, but this view faces axiological objections. A worry exists that cognitive enhancement undermines achievements and erodes intellectual character. Crucially, two parties seem to talk past each other because the nature of cognitive enhancement and the value of cognitive enhancement are not clearly distinguished. To end the stand‐off (...) between the two parties, I take insight from Gwen Bradford’s work and argue that, other things being equal, cognitive enhancement either decreases the value of cognitive achievement or undermines cognitive achievement. In the face of this threat, I further submit that we could learn to live with cognitive enhancement in an integrated way so that the lost value can be restored. (shrink)

The original proof of the four-color theorem by Appel and Haken sparked a controversy when Tymoczko used it to argue that the justification provided by unsurveyable proofs carried out by computers cannot be a priori. It also created a lingering impression to the effect that such proofs depend heavily for their soundness on large amounts of computation-intensive custom-built software. Contra Tymoczko, we argue that the justification provided by certain computerized mathematical proofs is not fundamentally different from that provided by surveyable (...) proofs, and can be sensibly regarded as a priori. We also show that the aforementioned impression is mistaken because it fails to distinguish between proof search (the context of discovery) and proof checking (the context of justification). By using mechanized proof assistants capable of producing certificates that can be independently checked, it is possible to carry out complex proofs without the need to trust arbitrary custom-written code. We only need to trust one fixed, small, and simple piece of software: the proof checker. This is not only possible in principle, but is in fact becoming a viable methodology for performing complicated mathematical reasoning. This is evinced by a new proof of the four-color theorem that appeared in 2005, and which was developed and checked in its entirety by a mechanical proof system. (shrink)

Against the standard interpretation of Kant's ‘Copernican revolution’ as the prioritization of epistemology over ontology, I argue in this paper that his critique of traditional metaphysics must be seen as a farewell to the perfectionism on which early modern rationalist ontology and epistemology are built. However, Kant does not simply replace ‘perfection’ with another fundamental concept of normativity. More radically, Kant realizes that it is not simply ideas but only the relation of ideas that can be subject to norms, and (...) thus he shifts the focus from the reality of ideas to the validity of judgments. Section 1 of this paper clarifies the pre-Kantian role of the concept of perfection and examines Kant's critical response to that concept. Section 2 identifies Kant's point of departure from the Cartesian ‘way of ideas.’ Section 3 explains the key problem of his novel account of epistemic normativity. I conclude that Kant's anti-perfectionism must be seen as the driving force behind his ‘Copernican revolution’ in order to fully appreciate his mature account of epistemic normativity. (shrink)

In the Metaphysical Foundations of Natural Science, however, motion plays a more significant role, and, concomitantly, it is in his writings on natural science that Kant most fully expresses his thoughts on the subject. For present purposes, the most relevant passage is paragraph 15 of the Preface of the Metaphysical Foundations, where Kant says that the concept of matter had … to be carried through all four of the indicated functions of the concepts of the understanding , where in each a new determination of this concept was added. ArgumentPost-Stalinist reforms resulted in dramatic changes in the ways of operation of Soviet science: one can say that they altered the very understanding of what science was, or should be, in the socialist society. A new vision came about as a result of political and rhetorical efforts of scientists, who pushed forward their various, often conflicting, agendas acting in accordance with specific rules of Soviet polity. The most visible part of the reform came with the 1961 administrative reorganization of the (...) USSR Academy of Sciences. The related series of changes, however, was much broader and comprehensive, modifying the relationships between science and ideology, politicians and academic researchers, and establishing the very division between fundamental and applied research, which had been strongly rejected during the preceding Stalinist period. How is time transformed when signs appear? In the sign system of the conditioned reflex, the sign (conditioned stimulus) reverses, changes the direction of time, and overcomes its unidirectionality and irreversibility. In a sense, there is a “return” to the past in the form of the future when the sign is introduced. The sign serves as a “Time machine” of sorts. The mechanism of time transformation is possible because a mirror is embedded inside the sign, the surface of which represents (...) the psychological present time. When past experience meets that surface in the present, it may be reflected in the form of a future event. In the course of reflection, time changes its direction: if earlier time flowed from the event (the cause), it subsequently flows to other events (the purpose). Previously inaccessible past experience is now projected into the present, “comes to life” in it and is transformed into the future. In humans, unlike in animals, signs of the second order appear. Inside the signs of the second order, a mirror is built in, and the experience of the past as a subject is reflected in the future form as an object. An individual has the opportunity to consider oneself externally as an object. It goes beyond the subjective form. Thus, secondary signs once again change time direction. Due to this phenomenon, time becomes doubly reversible: first it turns around in a conditioned stimulus, and then in a secondary sign. (shrink)

PA is Peano Arithmetic. Pr(x) is the usual Σ1-formula representing provability in PA. A strong provability predicate is a formula which has the same properties as Pr(·) but is not Σ1. An example: Q is ω-provable if PA + ¬ Q is ω-inconsistent (Boolos [4]). In [5] Dzhaparidze introduced a joint provability logic for iterated ω-provability and obtained its arithmetical completeness. In this paper we prove some further modal properties of Dzhaparidze's logic, e.g., the fixed point property and the Craig (...) interpolation lemma.
